Indications:
It is used in the treatment of catabolic or tissue reducing processes, Oxandrolone is indicated in conditions such as chronic infections, major surgeries (corticoid-induced myopathies, decubitus ulcers, burns) or severe trauma, in which it is necessary to interrupt the effects of catabolic processes or protein loss.
It is important to remember that these types of substances are adjuvants in these cases and do not replace the conventional treatment of these disorders.
In addition, it is used in the treatment of Turner Syndrome, to treat the short stature that occurs in these patients. Although the therapy is still controversial, recent experimental reports indicate that oxandrolone may be effective as a growth hormone and may enhance the effectiveness of growth hormone therapy.
Mechanism of action:
They reverse catabolic processes and negative nitrogen balance, promoting protein synthesis and stimulating appetite if there is simultaneous intake of calories and protein.
It is administered orally, like oxandrolone, has a first half-life of 0.55 hours and another of 9 hours. It is eliminated by the kidneys and in small amounts in the faeces.
Posology:
In adults, 2 to 4 weeks of therapy is usually adequate.
Doses in adults and adolescents, 2.5 mg twice or four times daily.
The maximum dose is 20 mg per day.
The dose in children is 0.25 mg per day.
In cases of Turner Syndrome, the dose is 0.05 to 0.125 mg/weight/day.
Generally, the patient should start and maintain the lowest effective dose to minimize adverse effects.
Contraindications:
Anabolic steroids are often used to treat symptoms associated with osteoporosis. However, prolonged use is not recommended due to the questionable effects of these agents and the risk of serious adverse effects is not warranted.
The use of anabolic steroids in athletes is not recommended.
Adverse effects:
In women, virilism, acne, clitoral enlargement, hoarseness, menstrual irregularities and weight loss are observed.
In prepubescent men, acne, penis enlargement, increased frequency of erections, abnormal hair growth.
In postpubertal men, bladder irritability (frequent urge to urinate), chest pain, breast enlargement, frequent erections.
Anemia, iron deficiency, loss of appetite, edema, nausea, vomiting, and unusual bleeding are seen less often.
Hypercalcemia (depression, habitual tiredness), rare darkening of the skin.
In very adult men, prostatic carcinoma or prostatic hypertrophy (difficulty or frequent urination) may be seen.
Hepatic necrosis (blackened stools, headaches, bloody vomiting, bad breath), liver cancer (stomach or abdominal pain, unexplained weight loss), peliosis hepatica (dark urine, loss of appetite, red patches on the skin) are rarely seen, mouth, nose, throat, fever, nausea, vomiting).
Drug interactions:
The use of an anabolic steroid with adenocorticoids, glucocorticoids, medications or foods that contain sodium may increase the possibility of edema.
Oxandrolone used in conjunction with anticoagulants may enhance the anticoagulant effect.
Anabolic steroids may lower glucose concentration, diabetic patients should be monitored for possible symptoms of hypoglycemia and the dose of hypoglycemic agents or insulin should be adjusted.
Simultaneous use of anabolic steroids with hepatotoxic drugs can increase hepatotoxicity, patients with liver disease should be carefully monitored.
Precautions and Warnings:
No in vitro or animal mutagenesis studies have been performed for oxandrolone.
Animal studies given 9 times the human dose have shown that oxandrolone can cause embryotoxicity, foetotoxicity, and infertility and increased masculinization, therefore it should not be used during pregnancy.
Use in children and adolescents should be done with caution due to possible premature closure of the epiphysis. Medication should be discontinued if no results are obtained after 6 months.
overdose:
Treatment for overdose is symptomatic and supportive. In case of overdose, it is treated with emesis and/or gastric lavage. Liver function should be monitored as hepatotoxicity is a potentially serious adverse effect. Go to the Medical Emergency Center.
